Full Job Description

Location: AVWA Offices

Reports to: Clinical Services Manager

Employment type: Full time/ Part time

Role purpose:

The Physiotherapist plays a critical role in supporting aged care clients and NDIS participants by conducting physical assessments, providing rehabilitation therapy, prescribing mobility aids, and collaborating with multidisciplinary teams to improve clients' physical health, safety, and overall well-being. The Physiotherapist will ensure services align with Aged Care Quality Standards and NDIS Practice Standards, participate in clinical audits, quality improvement initiatives, and professional development programs, and contribute to ongoing service improvements.

Key Responsibilities:

Aged Care Clients (HCP Program)

Conduct comprehensive mobility and physical function assessments to determine the client's needs.
Develop and implement individualised physiotherapy treatment plans to enhance mobility, reduce pain, and restore function.
Provide rehabilitation support for clients recovering from injuries, surgeries, strokes, and degenerative conditions.
Assess and prescribe mobility aids, assistive devices, and home exercise programs to improve independence.
Implement falls prevention strategies and educate clients, family carers, and care workers on risk reduction techniques.
Provide manual therapy, strength training, and pain management interventions.
Educate clients and family carers on safe movement, posture, and mobility techniques.
Collaborate with Care Managers, Occupational Therapists, Registered Nurses, and Care Workers to ensure integrated and holistic care planning.

For NDIS Participants

Conduct functional capacity assessments to support NDIS planning and goal setting.
Develop and implement tailored rehabilitation programs for NDIS participants with physical impairments.
Assess and recommend assistive technology, mobility aids, and home modifications to improve accessibility and functionality.
Provide therapy interventions for individuals with musculoskeletal, neurological, or developmental conditions.
Train NDIS participants, family carers, and support workers on the correct and safe use of assistive devices and therapy techniques.
Work collaboratively with Support Coordinators, Plan Managers, and allied health professionals to optimise service delivery.
Prepare detailed clinical reports, progress notes, and NDIS funding applications as required.

Home Assessments & Multidisciplinary Collaboration

Conduct home safety and mobility assessments and recommend modifications to enhance functionality.
Work closely with Occupational Therapists, Care Managers, Support Coordinators, and other allied health professionals to provide holistic care.
Provide education and training to clients and their families to ensure safe and effective use of physiotherapy interventions.
Deliver manual handling training sessions for care workers, focusing on safe lifting techniques, posture, and ergonomics.

Compliance, Quality Improvement & Professional Development

Ensure all physiotherapy interventions align with Aged Care Quality Standards and NDIS Practice Standards.
Participate in clinical audits, quality improvement initiatives, and registration audits to ensure compliance and service enhancement.
Maintain accurate and up-to-date clinical documentation in line with HCP and NDIS requirements.
Attend team meetings, training sessions, and professional development programs to stay updated on industry best practices.
Keep informed about changes in aged care and disability policies, funding models, and evidence-based practices.
Ensure compliance with all regulatory requirements, professional standards, and ethical guidelines.

Requirements:

Bachelor's degree in Physiotherapy.
Current AHPRA registration as a Physiotherapist.
Experience working with aged care clients (HCP) and/or NDIS participants.
Strong understanding of Aged Care Standards, NDIS Practice Standards, and funding frameworks.
Experience in functional assessments, rehabilitation therapy, mobility aid prescription, and exercise planning.
Ability to train and educate clients, family carers, and care workers on assistive technology and mobility equipment.
Experience in rehabilitation therapy for individuals recovering from injuries, surgeries, strokes, and degenerative conditions.
Proficiency in conducting home assessments and recommending modifications for safety and accessibility.
Excellent communication, problem-solving, and report-writing skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Valid driver’s license and willingness to travel for home visits.
NDIS Worker Screening Check (or willingness to obtain).

Good to have:

Experience with complex NDIS functional assessments and reports
Experience working with clients with neurological conditions (e.g., stroke, Parkinson’s, multiple sclerosis, autism, intellectual disability).
Proficiency in English and Vietnamese language skills is an advantage.
