About Supernova Medical Centre

Supernova Medical Centre is a busy, well-established bulk billing clinic, providing comprehensive and accessible healthcare to a diverse patient population. Our experienced GPs and allied health team work collaboratively to deliver high-quality, patient-centred care.

About the Role

This is an excellent opportunity for a Diabetes Educator looking to build a strong patient base within a high-demand clinic environment. You will work closely with our GPs and healthcare professionals to support patients living with diabetes through education, self-management strategies, and ongoing care.

We offer flexible working arrangements, an autonomous role with strong clinical support, and the opportunity to contribute to the growth of our diabetes services.

Key Responsibilities

As a Diabetes Educator, you will:

Provide comprehensive diabetes education, treatment, and management to adult and paediatric patients
Develop individualised care plans and self-management strategies
Educate patients on blood glucose monitoring, medications, insulin administration, nutrition, and lifestyle modifications
Support patients using diabetes technologies, including Continuous Glucose Monitoring (CGM) systems and insulin therapy where appropriate
Work collaboratively with GPs and allied health practitioners to deliver coordinated care
Liaise with patients, families, specialists, and other healthcare providers regarding patient management
Manage GP referrals and provide clear clinical reports and recommendations
Maintain accurate and timely clinical documentation
Promote diabetes prevention and healthy lifestyle practices within the community

Essential Requirements

To be successful in this role, you will have:

Credentialled Diabetes Educator (CDE) status or eligibility to obtain credentialling
Current AHPRA registration or relevant professional registration
Excellent communication and patient education skills
Strong clinical knowledge and confidence in diabetes management
Ability to work independently and efficiently within a busy clinical environment
A commitment to delivering exceptional patient care and customer service
A willingness to learn, grow, and contribute to service development

Desirable

Experience in private practice, general practice, or a medical centre setting
Experience with Medicare-funded Chronic Disease Management programs
Experience with diabetes technology, including CGM and insulin pump therapy
Familiarity with Best Practice or similar clinical software
