About the Role
This is an excellent opportunity for a Diabetes Educator looking to build a strong patient base within a high-demand clinic environment. You will work closely with our GPs and healthcare professionals to support patients living with diabetes through education, self-management strategies, and ongoing care.

Key Responsibilities
As a Diabetes Educator, you will:
- Provide comprehensive diabetes education, treatment, and management to adult and paediatric patients
- Develop individualised care plans and self-management strategies
- Educate patients on blood glucose monitoring, medications, insulin administration, nutrition, and lifestyle modifications
- Support patients using diabetes technologies, including Continuous Glucose Monitoring (CGM) systems and insulin therapy where appropriate
- Work collaboratively with GPs and allied health practitioners to deliver coordinated care
- Liaise with patients, families, specialists, and other healthcare providers regarding patient management
- Manage GP referrals and provide clear clinical reports and recommendations
- Maintain accurate and timely clinical documentation
- Promote diabetes prevention and healthy lifestyle practices within the community
Essential Requirements
To be successful in this role, you will have:
- Credentialled Diabetes Educator (CDE) status or eligibility to obtain credentialling
- Current AHPRA registration or relevant professional registration
- Excellent communication and patient education skills
- Strong clinical knowledge and confidence in diabetes management
- Ability to work independently and efficiently within a busy clinical environment
- A commitment to delivering exceptional patient care and customer service
- A willingness to learn, grow, and contribute to service development
Desirable
- Experience in private practice, general practice, or a medical centre setting
- Experience with Medicare-funded Chronic Disease Management programs
- Experience with diabetes technology, including CGM and insulin pump therapy
- Familiarity with Best Practice or similar clinical software
