The Connected Care Registered Nurse plays a critical role in delivering safe, high-quality virtual care through Remote Patient Monitoring (RPM) and Hospital in the Home (HITH), supporting patients to remain safely at home and avoid unnecessary hospital presentations.
This role is central to day-to-day clinical operations, including patient onboarding, remote monitoring, assessment and escalation of clinical risk, and coordination with medical and multidisciplinary teams. It directly supports timely intervention, continuity of care, adherence to clinical protocols, and responsive management of patients whose conditions require ongoing surveillance in the community.As the service continues to expand, this position will also support the pilot implementation of an enhanced 24-hour model of care, providing capacity to respond to after-hours clinical escalations for patients receiving RPM and HITH services. The model is being tested to ensure patients experiencing clinical deterioration or requiring additional support outside standard business hours can be safely assessed, managed, and coordinated within the community setting, reducing avoidable Emergency Department presentations and hospital admissions.
By enabling early discharge, reducing avoidable hospital utilisation, supporting patient flow, and facilitating timely escalation and intervention, this position contributes to improved patient outcomes, system efficiency, and patient experience. Hosted within the Triple I service, the role also supports the ongoing development, evaluation, and scalability of innovative, integrated models of care across Primary and Community Health, aligned with contemporary approaches to virtual care delivery and hospital avoidance.

You are an experienced and adaptable Registered Nurse with a strong clinical foundation and a passion for delivering care through innovative virtual models that support patients to remain safely at home.
You bring:
- Proven clinical assessment and critical thinking skills, with the ability to identify, assess and respond to patient deterioration, implement appropriate interventions, and escalate concerns in a timely manner.Experience working collaboratively within multidisciplinary teams to coordinate safe, patient-centred care across hospital, community and primary care settings.
- Strong communication and relationship-building skills, with the ability to engage patients, families and carers through virtual care platforms and support informed decision-making.
- Confidence using digital health systems and adapting to new technologies, including Remote Patient Monitoring (RPM) platforms and virtual models of care.
- The ability to manage clinical escalations, prioritise competing demands and make sound decisions in a dynamic and evolving service environment.
- A high level of accountability, organisation and time management, with the capacity to balance multiple patient needs while maintaining high standards of care.
- A commitment to innovation and service improvement, with an interest in contributing to the development and evaluation of emerging models of care, including Hospital in the Home (HITH), virtual care, and out-of-hours service delivery.
- You demonstrate a commitment to safe, high-quality care and are motivated to contribute to a growing service that is transforming how healthcare is delivered. You are comfortable working in an evolving environment and recognise the important role virtual care plays in supporting early discharge, reducing avoidable hospital presentations, responding to after-hours clinical escalation, and enabling patients to remain safely at home.
