Position Details
Position Title: Community Support Worker
Location: Rockingham WA 6168 and surrounds
Term: Full-time permanent
Salary: $78,578 per annum
Classification: Level 3—paypoint 2 (full-time)
Role Purpose
The Support Worker (Community) provides emotional, social, and practical assistance to individuals with challenging mental and health conditions. The role involves supporting participants in their daily living activities, fostering independence, and ensuring a high standard of care in line with organisational policies and procedures.
Key Responsibilities: General Duties
- Provide skilled assistance to high-care clients in the community by supporting ventilator use, managing phrenic nerve stimulator (PNS) devices, performing PEG feeding, and conducting suctioning procedures and assisting client with oxygen use.
- Manage and respond to high-care client behaviours with a person-centered approach, ensuring safety, dignity, and well-being while implementing appropriate behavior support strategies in line with care plans and best practices.
- Foster community inclusion by supporting clients in daily activities, social participation, and independent living.
- Assist with personal care, mobility, and access to community services.
- Facilitate social connections by accompanying clients to activities, appointments, and community events.
- Collaborate with community groups, welfare agencies, and service providers to enhance support networks.
- Assess client needs and provide tailored life skills training, health education, and digital literacy programs.
- Monitor client progress and advocate for access to essential resources, including healthcare and transport.
- Support individuals, including probationers in overcoming social, emotional, and financial barriers.
- Champion community-based solutions and raise awareness of available services to create a more inclusive environment.
Qualifications & Experience - Essential
- Diploma of Community Services or at least three years of relevant experience may substitute for the formal qualifications
- At least one year of relevant work experience in healthcare or disability support.
- Experience supporting, mentoring, or leading employees.
- Strong relationship-building skills with clients and colleagues.
- NDIS Screening Check
