Access Support Coordination is looking for a dedicated and experienced
Support Coordinator to join our growing team in
Hervey Bay, QLD. In this full-time position role, you'll support participants in implementing their NDIS plans, ensuring they understand their options and make confident, informed decisions. You’ll play a key role in coordinating supports while promoting participant capacity, independence, and effective use of funding.

Key Responsibilities
- Support participants to build meaningful and connected lives beyond funded supports
- Assist participants in understanding and implementing their NDIS plans
- Coordinate supports efficiently to maximise participant outcomes and funding utilisation
- Help gather supporting evidence that reflects changing participant needs and future funding requirements
- Identify opportunities in areas including employment, recreation, education, and housing
- Advocate for participants and liaise with mainstream services to ensure responsibilities are met
About You
- Proven experience in a Support Coordination role or similar within the Disability sector
- Qualifications in Disability, Community Services, Social Work, or a related field
- Strong knowledge of the NDIS and related processes
- Ability to manage KPIs and billable hour targets effectively (highly regarded)
- Excellent interpersonal and problem-solving skills with a participant-focused approach
- Current driver licence, Working with Children Check, and NDIS Worker Screening Clearance required
