An NDIS plan is only as good as the support behind it. That's where you come in.
As a Support Coordinator with Access Support Coordination in Hervey Bay, you'll work alongside participants to bring their NDIS plans to life — helping them understand their options, make confident decisions, and build a life that goes beyond funded supports. It's relationship-driven, it's purposeful, and the difference you make shows up in real moments of independence.
What you'll be doing
- Supporting participants to understand and implement their NDIS plans with clarity and confidence
- Coordinating supports efficiently to maximise participant outcomes and get the most from available funding
- Helping participants build meaningful, connected lives beyond what their plan covers
- Gathering supporting evidence that reflects changing needs and future funding requirements
- Identifying opportunities across employment, recreation, education and housing
- Advocating for participants and liaising with mainstream services to make sure responsibilities are met
You're the right person if...
- You have proven experience in a Support Coordination role or similar within the disability sector
- You hold a qualification in Disability, Community Services, Social Work or a related field
- You know the NDIS inside out — the processes, the funding, and how to navigate it for the people you support
- You can manage KPIs and billable hour targets without losing sight of the participant in front of you
- You communicate with warmth, think on your feet, and put participants first
- You hold a current Driver's Licence, Working with Children Check and NDIS Worker Screening Clearance
