Full Job Description

Experienced Mental Health Support Worker / Mentor (NDIS)

Location: Wyong, NSW
Hours: 4 days per week, 5 hours per day (20 hours per week)
Funding: Agency Managed NDIS Participant

Heart for Care is seeking a highly experienced and compassionate Support Worker/Mentor to provide community-based support to a male NDIS participant in his 30s living with complex psychosocial and cognitive disabilities. This can be either an employed role OR Sub-Contracted.

The participant has a diagnosis of Schizophrenia, Intellectual Disability, and a history of forensic mental health involvement. A Behaviour Support Practitioner has recently been engaged, and the successful support worker will work collaboratively with the participant's support team to promote stability, independence, community participation, and wellbeing.

About the Role

This is a rewarding opportunity to make a significant difference in someone's life through consistent, person-centred support. The participant can experience episodes of psychosis that may arise unexpectedly and can present risks to his safety and wellbeing. During these periods, he may be at increased risk of:

Missing important medications
Neglecting personal care and daily living tasks
Increased social isolation
Engaging in unsafe or high-risk behaviours
Involvement with emergency services or the justice system

The successful candidate will provide structured support, mentoring, community access, routine-building, emotional regulation support, and proactive monitoring of mental health and wellbeing.

Essential Requirements

Extensive experience supporting people with severe and persistent mental illness
OR
Formal qualifications in Mental Health, Psychology, Community Services, Social Work, Disability Support, or a related field
Demonstrated experience supporting individuals experiencing psychosis, schizophrenia, complex behavioural presentations, or forensic mental health challenges
Strong verbal communication skills and the ability to build rapport with participants and multidisciplinary teams
Mature, calm, and confident approach to crisis prevention and de-escalation
Ability to maintain professional boundaries while providing person-centred support
Current NDIS Worker Screening Check
Current Working with Children Check (if applicable)
Current First Aid and CPR Certification
Valid Driver's Licence and reliable vehicle
Ability to work collaboratively with Behaviour Support Practitioners and allied health professionalsHighly Desirable
Mental Health First Aid Certification
Experience implementing Positive Behaviour Support strategies
Experience working within forensic mental health or justice-related settings
Knowledge of recovery-oriented and trauma-informed practice
