Job description:

Deliver high-standard, person-centered nursing care to NDIS participants within their approved plan, adhering to clinical best practices and regulatory requirements.
Conduct comprehensive health assessments, develop and review health care plans in collaboration with participants, families, support staff, and health professionals.
Administer medications, manage wound care, monitor chronic health conditions, and respond to clinical emergencies.
Maintain accurate, objective, and timely clinical records and documentation in line with NDIS and organisational policies.
Provide education and clinical support to participants, their families, and support staff, promoting health literacy and participant choice and control.
Identify and report early and current indicators of discrimination, exploitation, neglect, abuse, violence, or other violations of participant rights.
Participate in Care Team Meetings, reviews, and clinical meetings to contribute to participant care planning and problem resolution.
Support the implementation of participant risk management and safe work systems for clinical services.
Work in partnership with other disability services, advocacy groups, allied health professionals, and community agencies to optimise participant outcomes.

Organisation Culture and Practice

Actively promote and reflect organisational values consistent with the Rights of Persons with Disabilities and NDIS principles.
Demonstrate cultural responsiveness and uphold participant dignity, choice, and control in all clinical interactions.
Collaborate with management and peers to enhance clinical service models, systems, and governance frameworks.
Contribute to workforce development by mentoring support workers on health-related matters and participating in clinical supervision and reflective practice.
Engage meaningfully with participants, families, and staff to inform them of improvements in clinical practice and service delivery.
Maintain up-to-date knowledge of contemporary disability and health care practices.

Establish Systems to Support Health and Manage Risk

Empower participants to understand and manage their health conditions while supporting informed risk-taking.
Identify and manage workplace safety and clinical risks, escalating concerns as per policy.
Maintain compliance with infection control, medication management, and incident reporting systems.
Deliver training and guidance to support workers in providing safe and effective participant care.
Monitor and contribute to continuous improvement in clinical systems, processes, and outcomes.

Qualification and Requirements

Mandatory - Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
Mandatory - Preferred qualifications in Disability, Community Health, Mental Health, or a related field.
Mandatory - Current Worker Screening Check and NDIS Worker Orientation Module completion.
Demonstrated understanding of the NDIS Practice Standards and Code of Conduct.
Valid driver's license

Experience

Experience providing person-centered, evidence-based nursing care, preferably in community, disability, or NDIS environments.
Strong understanding of clinical governance, risk management, and incident reporting systems.
Ability to work effectively within multidisciplinary teams.
Experience liaising with families, health professionals, and external agencies.
Skills in health assessments, care planning, medication management, and clinical documentation.

Skills

Excellent verbal and written communication skills.
High-level clinical reasoning, decision-making, and problem-solving abilities.
Strong organisational, time management, and prioritisation skills.
Ability to build trust and rapport with participants, families, and staff.
Proficient in Microsoft Office and clinical information systems.
Commitment to continuous professional learning and cultural responsiveness.
