Full Job Description

Care Well Connect is looking for a dependable Disability Support Worker to assist NDIS participants in the Essendon and Pascoe Vale areas.

This role would suit someone who enjoys working one-on-one with participants, can work independently, and understands the importance of consistency, dignity and choice.

About the role

As a subcontracted Support Worker, you may assist with:

Daily living tasks
Community access and appointments
Social and recreational activities
Building independence and life skills
Transport, where required
Meal preparation and light domestic tasks
Personal care, depending on participant needs and your experience
Following support plans and reporting relevant updates

About you

We are looking for someone who is:

Energetic, relatable and active
Able to build positive rapport with younger participants
Kind, patient and respectful
Punctual and consistent
Confident working independently
Comfortable supporting social and community engagement
Able to follow support plans and communicate clearly with the Care Well Connect team
Flexible with availability across weekdays and some weekends

Essential requirements

ABN
NDIS Worker Screening Check, current and valid
Working with Children Check, if applicable
Police Check, completed within the last 3 years or willing to obtain
First Aid certificate, completed within the last 3 years
CPR certificate, completed within the last 12 months
Driver’s licence and reliable vehicle
Comprehensive car insurance
Relevant experience in disability, mental health, aged care, youth work or community services
A Certificate III or IV in Disability, Individual Support or Community Services is preferred but not essential for the right applicant.

Contractor arrangement

This is a subcontractor position. Applicants must have, or be willing to obtain, an ABN and understand that they are responsible for their own tax, superannuation, invoicing and insurance obligations.

Hourly rate: $42.00 – $50.00 per hour, depending on experience, availability and participant support needs. Weekend or public holiday rates may be discussed where applicable.
