About Angel Care WA

Angel Care WA is a growing Western Australian care provider committed to delivering person-centred, culturally responsive and high-quality support to people with disability and members of culturally and linguistically diverse (CALD) communities.

We are expanding our NDIS services and are seeking an experienced and relationship-focused Support Coordinator & Community Partnerships Officer to join our team.

This is a strategic part-time position combining NDIS Support Coordination with community and professional relationship development. It is particularly suited to an experienced Support Coordinator seeking flexible hours and an opportunity to contribute to the growth of a developing service.

About the Role

Initially approximately 15 hours per week, the role will focus primarily on supporting a developing portfolio of NDIS participants while building productive relationships across the disability and community services sector.

As the participant portfolio grows, there is an opportunity for the successful candidate’s hours and responsibilities to increase.

Approximately 30–40% of the role will focus on Support Coordination, with the remaining time allocated to community engagement, stakeholder relationships and service development.

Key Responsibilities

NDIS Support Coordination

· Support participants to understand and effectively implement their NDIS plans.

· Assist participants to identify suitable providers and access appropriate mainstream, community and funded supports.

· Build participants’ capacity to exercise choice and control over their supports.

· Coordinate services across disability, allied health, community and mainstream providers.

· Monitor participant budgets and utilisation of funded supports.

· Identify potential service gaps and respond proactively to changing participant circumstances.

· Maintain accurate case notes, participant records and supporting documentation.

· Prepare progress reports and other documentation required by participants, plan managers and the NDIA.

· Support participants through plan reassessments and changes where appropriate.

· Maintain appropriate professional boundaries and manage actual, potential or perceived conflicts of interest.

Community Partnerships and Stakeholder Engagement

· Develop professional relationships with Support Coordinators, plan managers, allied health professionals, community organisations and other sector stakeholders.

· Represent Angel Care WA professionally within relevant community and disability-sector networks.

· Identify opportunities for collaboration and community partnerships.

· Assist Angel Care WA to strengthen its engagement with CALD communities across Perth.

· Contribute practical market insights to the development of Angel Care WA’s NDIS services.

· Participate in relevant networking and community engagement activities.

About You

We are particularly interested in candidates who combine strong NDIS knowledge with established professional relationships within the Perth disability sector.

The successful candidate will ideally demonstrate:

· Previous experience in NDIS Support Coordination, case management or a closely related role.

· Strong understanding of NDIS plans, funding categories and participant-centred service delivery.

· Knowledge of NDIS Practice Standards, participant choice and control, privacy and conflict-of-interest requirements.

· Excellent communication, relationship-management and stakeholder-engagement skills.

· Ability to work independently and manage a participant portfolio effectively.

· Strong documentation, reporting and organisational skills.

· A professional network within the Perth NDIS, allied health or community services sector will be highly regarded.

· Experience working with culturally and linguistically diverse communities will be highly regarded.

Relevant qualifications in Community Services, Disability, Social Work, Psychology, Allied Health, Human Services or a related discipline will be well regarded.

Essential Requirements

The successful applicant must hold or be willing to obtain all relevant employment and NDIS-related screening requirements applicable to the position, including:

· NDIS Worker Screening Check, where applicable.

· National Police Clearance.

· Current Australian driver’s licence and access to reliable transport where required.

· Appropriate working rights in Australia.

· Relevant professional or employment checks required for the role.
