Disability Support Worker (Male & Female) – Complex Care & Personal Care experienced

Tree of Goals provides professional support to people from diverse backgrounds. We assist with everyday living activities, including personal care, household tasks, shopping, and meal preparation. Our focus is on building skills, promoting independence, and helping participants achieve their full potential.

About the Role

As a Disability Support Worker, you will deliver person-centered support to individuals with disabilities, including those with complex behaviours and personal care needs.

You will play a key role in promoting:

Independence
Community inclusion
Decision-making
Personal choice

Support will always be tailored to each participant’s individual goals and needs.

Key Duties & Responsibilities

Support participants with daily living activities, including personal care and household tasks
Assist with community access, outings, and social participation
Provide support in line with individual care plans
Monitor participant safety and wellbeing and report concerns to management
Deliver 1:1 or small group support based on individual needs
Maintain accurate documentation, including case notes and communication records
Contribute to individual support plans and track participant progress
Work collaboratively with families, carers, case managers, and allied professionals
Assist in developing and implementing behaviour support programs
Act as a key worker when required, including preparing monthly progress reports
Communicate effectively with team members to ensure consistent service delivery
Participate in team meetings, case reviews, and planning processes
Provide support to other staff when needed
Follow incident management policies and report all incidents appropriately
Maintain records and documentation using Shiftcare software
Keep certifications and compliance documents up to date

About You

Essential:

Minimum 1 year experience as a Disability Support Worker
Experience with personal care and/or complex behaviours
Right to work in Australia
Valid Driver Licence

Preferred (or willingness to obtain):

Certificate III in Individual Support, Aged Care, or equivalent
NDIS Orientation Module
First Aid & CPR Certificate
Working With Children Check
NDIS Worker Screening Check
National Police Check
National Hand Hygiene Certificate
100 points of ID
