The Community Assessment and Response Proactive Team are recruiting a Part-Time Credentialled Diabetes Nurse Educator.
About the Role
There is an opportunity for a part time permanent position as a Credentialled Diabetes Nurse Educator working at Monash Health community sites including Cranbourne Community Hospital.
The Community Assessment and Response Proactive Team is a large multi-disciplinary team covering multiple community sites across campuses including Pakenham, Cranbourne Community Hospital, Springvale, Berwick and Dandenong Community Health and works closely with the Monash Health Diabetes Unit. Your primary focus within this role is to provide diabetes education to patients in the community setting and liaison with all members of our friendly and supportive multi-disciplinary team. Specifically, you will provide evidence-based, patient-centred, specialised care and education for patients in the community.
Key functions of the role:
Delivering assessment, consultation and treatment
Managing complex cases
Coordinating clinical care and providing clinical supervision or leadership as required
Working collaboratively with multidisciplinary teams
Contributing to strategic planning, team performance, professional development andservice improvement activities to continually develop an innovative contemporary health service
About You
Qualification / technical skills / knowledge / experience:
Minimum: Post Graduate Certificate of Diabetes Education (Australian Diabetes Education Association accredited)
Australian Diabetes Educators Association (ADEA) Credentialed Diabetes Educator
Minimum 3 years’ experience in diabetes education (highly desirable)
