The Homes First Program is an exciting, multi‑year pilot initiative designed to support single adults, families, and young people who are experiencing homelessness or rough sleeping. Our goal is simple yet powerful: secure safe, long‑term housing first, then wrap around the flexible, culturally safe and trauma‑informed supports needed for individuals and families to thrive.

The program spans three years of case management, with an additional year of intermittent support to ensure ongoing stability. The outcomes we strive for include long‑term housing, improved mental and physical health, and strengthened connection to community.

Together, we work toward a future where homelessness is rare, brief, and non‑recurring.
We are seeking several compassionate, skilled, and values‑driven Homes First Case Manager to support individuals and families to secure and maintain safe, sustainable housing. This role is part time (0.8 FTE) 60.80 hours or full time (1.0 FTE) 76 hours per fortnight on a fixed term basis till June 2027.
In this role, you will:

Provide flexible, trauma‑informed, culturally safe case management

Complete comprehensive needs assessments and develop client‑led support plans

Offer practical support to individuals and families, including accompanying children

Connect clients with our multidisciplinary team and external service providers
-
Support clients to strengthen daily living skills where there are service gaps
-
Promote self‑determination, especially for Aboriginal clients, including connection to Country and community
-
Champion a recovery‑focused, harm‑reduction approach
-
Work collaboratively to prevent future episodes of homelessness
