Working within a connected multidisciplinary team, you’ll support older people transitioning into community or residential aged care supports. Here, the focus is on patient-centred care, meaningful connection and seeing the impact of your work over time.
CO-ACT at Hampstead focuses on the unique pace and environment at Hampstead, where nurses have more opportunity to:
- Build rapport with patients and families
- Deliver connected, person-centred care
- Work collaboratively across nursing, allied health and support services
- Contribute to recovery and long-term outcomes
- Grow their career within a supportive team environment
Set within the leafy surrounds of Hampstead, this is a workplace that offers a calmer and more considered pace of care—without losing the complexity, collaboration and professional fulfilment that makes nursing meaningful.
