Are you a GP looking for meaningful, flexible clinical work — without the commute?

Veteran Pathways & Proactive Care is seeking experienced General Practitioners for a fully remote telehealth position, delivering structured case conferencing and chronic disease management for veterans and patients with complex, multi-system health needs.

This is not a standard telehealth locum. You will provide clinical leadership within integrated Chronic Care Teams, working collaboratively with allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful, and genuinely multidisciplinary.

We work across both Medicare and DVA billing streams, with a dedicated focus on veteran health and chronic disease management. If you’re based in Sydney, the Central Coast, Newcastle, or anywhere across NSW with a reliable internet connection, this role offers the flexibility to work from home while doing clinical work that makes a real difference.

Remuneration

First 3 Months (Guaranteed) = $240.00 per hour

After 3 Months = 70% of billings

Structured as an independent contractor arrangement

Bulk billing model across Medicare and DVA billing streams
Sign-on bonus and anniversary incentive available (details on enquiry)
Central Coast and Newcastle are DPA-eligible locations — GPs with a 19AB exemption, including international medical graduates, are welcome to apply

Location

This is a fully remote telehealth position. There is no physical clinic attendance required.

We welcome applications from GPs based anywhere across NSW, including the Sydney metropolitan area, Central Coast, Newcastle, and regional NSW. All you need is a suitable home-office setup and a reliable internet connection.

GPs based on the Central Coast or in Newcastle may be eligible for additional DPA-related benefits, including pathways for those holding a 19AB exemption.

The Role

As a General Practitioner within our Chronic Care Team model, you will provide clinical leadership across structured telehealth case conferencing and patient review sessions:

Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams
Provide medical oversight and clinical direction for patients with complex chronic and multi-system conditions
Integrate findings from clinical assessments, pathology, imaging, and allied health input into coordinated, evidence-based care plans
Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPCCMPs) and Team Care Arrangements (TCAs)
Collaborate with allied health professionals, specialists, and care coordinators to optimise patient outcomes
Deliver telehealth consultations for patient follow-up, clinical review, and ongoing management
Manage prescribing, referrals, and investigation requests as part of the coordinated care model
Ensure clinical safety, prioritisation, and escalation pathways are clearly identified and actioned

Working Pattern: Case conferencing sessions are typically scheduled in structured blocks, with approximately 22 patient reviews conducted over an 8-hour session. Flexible scheduling is available, and sessions are conducted entirely via telehealth platforms.

Veteran Health & DVA Focus

Veteran Pathways & Proactive Care has a dedicated focus on veteran health and chronic disease management for both Medicare and DVA-funded patients. You will work with veterans who often present with complex, multi-system conditions including chronic pain, mental health comorbidities, musculoskeletal injuries, and the long-term effects of military service.

Experience with DVA billing, the Coordinated Veterans’ Care (CVC) Program, or veteran-specific health assessments is highly regarded but not essential — comprehensive onboarding and support will be provided.

Requirements

Essential

Medical degree (MBBS or equivalent)
Current registration with the Medical Board of Australia (AHPRA)
Fellowship with RACGP or ACRRM (VR GP), or eligibility for 19AB exemption in a DPA location (Central Coast, Newcastle)
Experience in chronic disease management, preventative health, and complex care coordination
Competence in prescribing, referrals, and investigation management
Strong communication and collaboration skills within multidisciplinary teams
A suitable home-office environment and reliable internet connection for telehealth delivery

Desirable

Experience with telehealth consultations and digital health platforms
Experience working within team-based or multidisciplinary care models
Interest in or experience with veteran health, DVA processes, or the CVC Program
Experience with GP Management Plans (GPCCMPs), Team Care Arrangements (TCAs), and chronic disease care planning

Guaranteed Hourly Rate — $240/hr for the first 3 months, moving to 70% of billings thereafter
