Unmatched Support Services is hiring a Casual/Temporary Care Coordinator role in Gold Coast, QLD and Brisbane, QLD. Apply now to be part of our team.

We are seeking a compassionate, organized, and proactive Care Coordinator to join our team in the Gold Coast. This is an exciting opportunity for someone who is passionate about delivering high-quality care and making a meaningful difference in the lives of participants.

About the Role

As a Care Coordinator, you will be responsible for coordinating care services, supporting participants, and ensuring they receive quality, person-centred support. You will work closely with participants, families, support workers, and allied health professionals to develop and manage individualized care plans.

Key Responsibilities

Coordinate and manage participant care and support services
Develop, implement, and review individualized care plans
Maintain regular communication with participants, families, and stakeholders
Schedule and coordinate support services efficiently
Monitor participant progress and ensure quality outcomes
Maintain accurate documentation and case notes
Ensure compliance with NDIS standards, policies, and procedures
Build positive relationships with participants and the broader care team

About You

The ideal candidate will have:

Experience in care coordination, case management, disability support, aged care, or community services
Excellent communication and interpersonal skills
Strong organizational and time management abilities
Knowledge of the NDIS (preferred)
Ability to manage multiple priorities in a fast-paced environment
Strong problem-solving skills and attention to detail
Current Driver's Licence and reliable vehicle
Current NDIS Worker Screening Check (or willingness to obtain)
Working With Children Check (if required)
Current First Aid & CPR Certificate (preferred)
