We are seeking a compassionate, organized, and proactive Care Coordinator to join our team in the Gold Coast. This is an exciting opportunity for someone who is passionate about delivering high-quality care and making a meaningful difference in the lives of participants.
About the Role
As a Care Coordinator, you will be responsible for coordinating care services, supporting participants, and ensuring they receive quality, person-centred support. You will work closely with participants, families, support workers, and allied health professionals to develop and manage individualized care plans.
Key Responsibilities
- Coordinate and manage participant care and support services
- Develop, implement, and review individualized care plans
- Maintain regular communication with participants, families, and stakeholders
- Schedule and coordinate support services efficiently
- Monitor participant progress and ensure quality outcomes
- Maintain accurate documentation and case notes
- Ensure compliance with NDIS standards, policies, and procedures
- Build positive relationships with participants and the broader care team
About You
The ideal candidate will have:
- Experience in care coordination, case management, disability support, aged care, or community services
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Knowledge of the NDIS (preferred)
- Ability to manage multiple priorities in a fast-paced environment
- Strong problem-solving skills and attention to detail
- Current Driver's Licence and reliable vehicle
- Current NDIS Worker Screening Check (or willingness to obtain)
- Working With Children Check (if required)
- Current First Aid & CPR Certificate (preferred)

This role is Casual with the possibility of transitioning to Part-time as the workload increases.
