About the Role:

Join our Mental Health Rehabilitation Unit as a Rehabilitation Support Worker, supporting individuals, and their families, in their recovery and accomplishment of individual goals.

In this role, you will provide direct support to consumers experiencing mental health challenges, helping them develop skills for independent living and community participation. Our collaborative team environment focuses on person-centred care and recovery-oriented practice.

Major responsibilities include:

providing practical support with daily living activities and skill development
assisting with group programs and therapeutic activities
supporting consumers in accessing community resources and services
contributing to care planning and documentation
working collaboratively with multidisciplinary teams

The Modbury Mental Health Rehabilitation Unit (MMHRU) offers contemporary, evidence based, and high-quality rehabilitation services through a multidisciplinary team committed to person-centred, trauma-informed care. Our collaborative environment focuses on compassionate, solution-focused approaches that promote skill development, social inclusion, and empower consumers to achieve their recovery goals. You'll work closely with Wondakka Community Rehabilitation Centre and The Gully services, contributing to mental health rehabilitation innovation while fostering continuous improvement and co-design practices.

Consumers at the MMHRU are voluntary and their rehabilitation program is developed based on their individual goals and needs.

The purpose of this position is to provide support to consumers in the least restrictive environment possible through the provision of effective psychosocial support and rehabilitation activities in order to successfully enable the transition to community living and to support individual recovery.

You’ll be confident managing your time and resources efficiently, and comfortable working both independently and as part of a collaborative, multi‑disciplinary team.
