The Specialist Palliative Care Service provides outreach and inpatient services across the Central Coast region and has hubs across the district including Long Jetty Health Centre, Gosford and Wyong Hospitals. You will play a key role in clinical governance and providing support to clinical teams delivering palliative and end of life care in non-specialist settings. This will include support to patients’, families and staff in hospital wards, patients’ homes, and Residential Aged Care Homes (RACHs).

About The Opportunity

The Central Coast Local Health District (CCLHD) is seeking a dynamic and experienced Clinical Nurse Consultant Grade 2 to join the Specialist Palliative and End of Life Care Service. This permanent full-time opportunity is ideally suited to a motivated clinician who thrives in a collaborative environment and is committed to delivering high-quality, person-centred care to patients and families during the final stages of life.

In this role you will:

Improve safety and quality through evidence based practice, education, clinical service planning, change management and expert consultancy with both internal and external stakeholders.
Role model advanced nursing practice by demonstrating high level clinical knowledge, skills, experience and judgement.
Lead quality and risk initiatives that embed contemporary evidence and support nurses and teams to deliver appropriate clinical care.
Provide expert clinical advice and deliver specialty education programs that build capability and strengthen safe, high quality care.
Plan, implement and evaluate care aligned with evidence and recognised standards.
Collaborate with consumers and teams to design and deliver safe, effective and efficient models of care consistent with Central Coast Local Health District priorities.
Assess contemporary issues in service delivery, clinical practice, research and policy, and recommend appropriate actions.
Develop and evaluate role specific plans to support ongoing service improvement.
Provide timely reports and recommendations that maintain nursing standards and guide future service direction.
Assist in the collection, management and reporting of data related to activity-based funding across the district as well as clinical outcome measures including the Palliative Care Outcome Collaborative (PCOC).
Take an active role in relevant governance committees including the LHD Advanced Care Planning End of Life Committee, Drugs and Therapeutics Committee, NSW Health Sub and Non Acute Care Working Group focusing on activity based management, as well as departmental Mortality and Morbidity meetings, Research and Clinical Practice Committee, and other meetings as required.

About You

We are looking for someone who:

Holds a current AHPRA registration as a Registered Nurse (Division

, with at least five years’ post registration experience, including three years in the specialty, relevant postgraduate qualifications, and a current unrestricted NSW driver’s licence.
Demonstrates leadership skills that support the ongoing development of evidence-based clinical practice.
Can work effectively within acute and primary health care service delivery models, both independently and as part of a multidisciplinary team.
Has highly developed written and verbal communication skills, including negotiation and conflict resolution abilities.
Can initiate and contribute to planning, quality improvement initiatives, and research activities.
Has experience in education, clinical leadership, mentoring, and staff supervision.
