Are you an experienced and compassionate social worker ready to make a difference in the lives of children and their families during times of illness, crisis and/or trauma? Join our dynamic team at Central Coast Local Health District (CCLHD) as a Community Paediatric Social Worker and be a crucial part of delivering optimal health outcomes and psychosocial care.

Based at Wyong Hospital, this position enables you to further your current knowledge and gain a broad breadth of specialist community based paediatric social work experience.

About the Opportunity

This position is part of the Community Paediatric multidisciplinary team based at Wyong Hospital that provides a professional social work service to children, and their parents/carers/families to enable optimal health outcomes consistent with CCLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

In this position you will:

Conduct comprehensive psychosocial assessments for children and their families experiencing disability, illness, trauma and/or crisis.
Deliver crisis management and short-term interventions tailored to individual child and family needs.
Provide counselling, emotional support and practical strategies to assist families in responding to diagnosis, disability, acute/chronic illness or traumatic events.
Apply trauma-informed, child-centred, person-centred and culturally responsive practice approaches.
Undertake case planning, service coordination, advocacy and referrals to appropriate internal and external supports.
Liaise effectively with children, parents/carers and extended family members to strengthen coping and resilience.
Adapt communication styles to suit diverse developmental, cultural and psychosocial needs.
Collaborate closely with multidisciplinary team members and senior managers to deliver integrated care.
Partner with internal departments and external agencies to ensure proactive, high-quality psychosocial support within a community-based service.
Manage competing priorities effectively while maintaining a high standard of professional social work practice.

About You

We are looking for someone who has:

A tertiary qualification in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to the Community Paediatric Social Work position.
Knowledge of providing social work services in a health service setting with an interest in the area of paediatrics, disability, grief and bereavement and knowledge of social work psychosocial assessment processes and its application in practice.
Knowledge of the psychosocial impact of acute and chronic health conditions upon children and their family/carers and knowledge of social work interventions and their application within an outpatient community service setting.
