We are seeking a dedicated Clinical Nurse to join the Transition Care Program (TCP) — a service that makes a genuine difference to older people navigating the step between hospital and home.
Based across Adelaide's northern metropolitan region, this part-time ongoing role places you at the heart of person-centred care, supporting clients to regain independence and confidence after an acute hospital stay.
About the Transition Care Program (TCP)
The Transition Care Program is at the heart of modern, patient-centred healthcare — supporting older patients at a critical point in their journey.
The Transition Care Program provides a range of short-term care services to eligible older people following a hospital stay to help them regain as much independence as possible.
Services include therapy, nursing support and /or personal care that will help eligible older people regain as much independence as possible and assist them in making long term care and support arrangements.
About the Role:
As a Clinical Nurse (RN2C), you will provide advanced, person-centred nursing care while also playing a key role in coordination, leadership and clinical decision-making.
You will coordinate and deliver clinical nursing care for TCP clients in community and residential settings. Your core responsibilities will include:
- Conducting comprehensive nursing assessments and developing individualised goal-directed care plans
- Collaborating with a multidisciplinary team — including allied health, GPs, and aged care providers — to optimise each client's transition outcomes
- Monitoring, evaluating, and adjusting care in response to changing client needs across the program episode
- Supporting clients and their families with education, advocacy, and connection to ongoing community services
- Providing clinical leadership and support to less experienced staff
- Contributing to quality improvement, research and service development activities

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.

About You:
You’re an experienced and confident Registered Nurse who thrives in dynamic, team-based environments.
You bring:
- At least 3 years post-registration experience
- A passion for supporting patients through recovery and transition
- Strong clinical skills and the ability to manage complex patient needs
- Strong assessment skills with the ability to work autonomously in community environments
- Demonstrated experience in aged care, rehabilitation, or community/primary health nursing
- Sound knowledge of transition care principles and the aged care service landscape in South Australia
- Effective communication skills to engage clients, families, and multidisciplinary colleagues
This role involves delivering care in community and patient home settings as part of the Transition Care model. A current unrestricted Australian driver’s licence is essential to your ability to work in community settings.

NALHN provides a range of hospital (inpatient), outpatient, community and home-based services across multiple sites in Adelaide’s north and north-eastern suburbs, including Aboriginal health, mental health, and GP services.
Lyell McEwin Hospital – the major hospital in northern Adelaide for emergency care, complex and multi-day surgery, medicine, obstetric, paediatrics and outpatient services.
Modbury Hospital – a general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.
Mental Health Services – adult and older persons mental health services and statewide forensic mental health services, across hospital and community settings.
Watto Purrunna Aboriginal Primary Health Care Services – provides culturally responsive primary healthcare services for Aboriginal and Torres Strait Islander people in-hospital and across four community sites.
Primary Health Care Services – including GP Plus sites at Elizabeth, Modbury and Gilles Plains.
