Our team works closely with participants, families, and communities to create meaningful outcomes and promote inclusion, confidence, and everyday living skills.

About the Role

As a Community Support Worker with Psychosocial Partner, you will play an important role in supporting people living with mental health conditions to achieve greater independence and improve their overall well-being. Based in Adelaide, this is a full-time position where you will provide client-centred support tailored to individual needs, assisting participants with daily living activities, community engagement, social connection, and achievement of their personal goals. This role requires compassion, reliability, and a genuine passion for making a positive difference in the lives of others.

Key Duties and Responsibilities

· Meeting with clients to understand their needs and create personalised support and care plans

· Providing community support to clients living with disability

· Communicating with clients to better understand any social, emotional, behavioural or well-being challenges they may be facing

· Keeping track of client progress, updating case notes and preparing reports when needed

· Connecting clients with external services and organisations for extra support and specialised assistance

· Identifying community needs and available services related to housing, health, education, employment and welfare support

· Working closely with community groups, government agencies, and service providers to improve access to support services

Preparing reports and maintaining accurate case documentation by recording and managing client information and data for ongoing support and service delivery.

Skills and Experience

The ideal candidate will demonstrate:

Strong communication and interpersonal skills
Empathy, patience and a genuine passion for helping others
Ability to work effectively both independently and within a team
Excellent organisational and documentation skills
Understanding of community services, case management and client support practices
Ability to maintain confidentiality and professional boundaries

Essential Requirements

Qualification in Community Services, Disability Support, Social Work, Youth Work, Welfare Studies or related field
NDIS Worker Screening Check
Working With Children Check
National Police Clearance
Current Driver’s Licence and reliable transport
