Your opportunity

As a Care Partner Support Officer, you’ll play a vital role in supporting the Care Partner team through administration, coordination, and client caseload management.

Acting as a primary point of contact, you will provide exceptional customer service while supporting the delivery of care under the Wellness Model of Care.

You’ll make an impact by

Acting as a key contact for clients, families, and advocates
Supporting Care Partners with coordination, scheduling, and administrative tasks
Managing a client caseload while monitoring wellbeing and outcomes
Maintaining accurate, timely documentation across systems
Collaborating with the wider team to ensure continuity of care and positive client experiences

What you’ll bring

To succeed in this role, you’ll need the following qualifications:

Diploma in Community Services Coordination, Certificate III in Aged Care, or equivalent
National Police Check
Current Australian Driver’s Licence

We would also love you to bring the following skills:

Strong relationship-building skills and a genuine ability to connect with clients and families
Empathy and respect for people from diverse backgrounds
Excellent communication and customer service skills
Strong organisational skills and attention to detail
The ability to manage competing priorities and meet deadlines
A collaborative mindset and willingness to support team outcomes
Initiative and a proactive approach to problem-solving
