As a Care Partner Support Officer, you’ll play a vital role in supporting the Care Partner team through administration, coordination, and client caseload management.
Acting as a primary point of contact, you will provide exceptional customer service while supporting the delivery of care under the Wellness Model of Care.
You’ll make an impact by
- Acting as a key contact for clients, families, and advocates
- Supporting Care Partners with coordination, scheduling, and administrative tasks
- Managing a client caseload while monitoring wellbeing and outcomes
- Maintaining accurate, timely documentation across systems
- Collaborating with the wider team to ensure continuity of care and positive client experiences
What you’ll bring
To succeed in this role, you’ll need the following qualifications:
- Diploma in Community Services Coordination, Certificate III in Aged Care, or equivalent
- National Police Check
- Current Australian Driver’s Licence
