The Team & Role
Join the Peninsula Care Group Access team, the first point of contact for community health, rehabilitation, ageing, pain, palliative, and specialist services. In this customer-focused role, you will support clients through the referral and intake process, provide information about available services, assess eligibility and service needs, coordinate appointments, and ensure accurate referral management and data entry. Working as part of a collaborative team, you will play an important role in connecting clients with the right care at the right time.

Skills & Experience Required
A relevant tertiary qualification and/or significant relevant experience in the health or community health services sector
Advanced interviewing and engagement skills
Proficiency in the Microsoft Office suite, particularly MS Word and other data base software programs
Demonstrates advanced computer and keyboard skills
Highly developed verbal and written communication skills
