What you'll be doing
As a Supportive and Palliative Care Nurse Specialist, you will provide advanced clinical leadership and care coordination for people with complex, life-limiting illnesses. You will play a pivotal role in supporting patients and carers while helping clinical teams deliver high-quality, consistent palliative care.
Lead specialist nursing practice, clinical assessment, care planning and case management for patients with complex needs.
Coordinate care across acute, primary and community services to support seamless transitions and continuity of care.
Lead quality improvement and evidence-based initiatives that strengthen clinical practice and patient outcomes.
Provide expert advice, education, mentorship and clinical leadership to nurses and multidisciplinary colleagues.
Support patients to achieve their end-of-life care wishes, including care in their preferred location wherever possible.

Selection Criteria:
Demonstrated at least three ( years full time equivalent post registration experience in Palliative Care, Cancer Care Services, Advanced Nursing or clinical experience deemed appropriate by the employer
Post graduate qualifications in Palliative Care, Cancer Care or Advanced Nursing
Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning
Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Policies and National and State Standards related to Palliative Care
Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations, including information technology platforms
Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based service
NSW Driver's licence and willingness to use for work purposes
