Full Job Description

Elderbloom is currently seeking experienced Therapy or Physiotherapy Assistants to join our Casual Pool, working across all our facilities, Wanneroo Nursing Home and Jacaranda Lodge in Wanneroo and Barridale Lodge in Kinglsey.

Elderbloom Community Care Centres is a not-for-profit organisation situated in the northern suburbs and we offer a range of high-quality aged care accommodation and support services.

At Elderbloom we are dedicated to the quality of our services and programmes, the development of our people, leadership and organisational development, whilst maintaining financial management and sustainability.

In collaboration with our residential care facility management, the successful candidate will be responsible for the provisions of quality occupational therapy assessments, interventions and advice in accordance with residents assessed needs and wishes and all legislative requirements.

Your main duties will include:

Comply with the Aged Care Quality Standards and organisational policies and procedures.
Work closely and collaboratively with a team of Clinical and Registered Nurses, Enrolled Nurses, Allied Health professionals, Care Workers, and other Therapy Assistants, complete and review designated lifestyle assessments, as per the agreed assessment schedule and changes to resident clinical needs and wishes.
Participate in multi-disciplinary team meetings to finalise review and evaluate resident care plans as per agreed schedule.
Deliver and record a planned calendar of leisure and lifestyle activities, encouraging residents to attend.
Complete all resident progress notes and charting accurately, in a professional and timely manner.
Respond effectively to residents changing needs e.g. returning from hospital, complex clinical needs, palliative, or terminal care.
Liaise and work with external health providers e.g. General Practitioners, Allied Health professionals, acute hospitals, and other health agencies to deliver high quality therapy and lifestyle services to residents.

To be a successful applicant you will have:

A Therapy qualification listed below or alike:
Certificate III in Leisure and Health
Certificate IV in Allied Health Assistance
Certificate IV in Physiotherapy Assistance
At least 1 year of experience working in Aged Care
Demonstrated experience in evidence based and resident-centred practice
Intermediate computer literacy (MS Office)
Excellent verbal and written communication skills
Organisational and time management skills
Police Clearance valid as of the last 6-months
The right to work in Australia
