We are seeking a Transition Facilitator to join The Arches team within the Bail Accommodation Support Program (BASP). This is a unique opportunity to support people experiencing homelessness and multiple, complex needs as they transition from short-term accommodation into stable, longer-term housing.
Working alongside participants, you’ll build trusting relationships and provide person-centred, strengths-based support tailored to their individual circumstances. You’ll work with participants to identify barriers to housing and wellbeing, connect them with appropriate services and supports, and help them develop the skills and connections needed to sustain their accommodation and independence.
This is a full-time, maternity leave contract until 26th February 2027 based at Port Adelaide.
You’ll be trusted to make a valuable impact by:
- Providing case management and practical support to participants experiencing homelessness or housing instability, supporting their transition into suitable longer-term accommodation.
- Building trusting relationships with participants experiencing multiple and complex needs, which may include mental health, substance use, family violence, financial hardship, disability and other challenges.
- Developing individualised support plans, undertaking risk assessments and identifying barriers that may impact a participant’s ability to secure and sustain housing.
- Supporting participants to strengthen their wellbeing, community connections and independence, while building their capacity to maintain stable accommodation.
To thrive in this role, you will have:
- Tertiary qualifications in Social Work, Community Services or a related field, or significant relevant experience.
- Knowledge of the community services sector in South Australia with a specific emphasis on homelessness and the criminal justice sectors.
- Demonstrated experience supporting people who are homeless or at risk of homelessness.
- High level of cultural awareness and commitment to working respectfully with Aboriginal communities.

- Experience working with individuals with complex needs, including those involved in the criminal justice system.
