The Role
We are looking for an experienced and passionate Chronic Disease Management Nurse to join our Elizabeth Medical Centre.
Working closely with GPs, nurses, specialists and allied health professionals, you will provide patient-centred chronic disease management and preventative healthcare services. You’ll prepare care plans, conduct health assessments, coordinate multidisciplinary support and empower patients to take greater ownership of their health.

Your Responsibilities
- Prepare GP Management Plans and Team Care Arrangements in collaboration with patients and GPs.
- Gather patient histories, biometrics and information relating to chronic conditions.
- Help patients set achievable health goals and develop practical action plans.
- Coach and motivate patients to make sustainable health and lifestyle changes.
- Conduct preventative health assessments, including heart health, diabetes risk, 45–49 and 75+ assessments.
- Complete health assessments for Aboriginal and Torres Strait Islander peoples, veterans, refugees and people with intellectual disability.
- Provide ongoing monitoring and support to eligible patients.
- Perform clinical services such as ECGs, spirometry, ABI testing and urinalysis.
- Coordinate care between GPs, specialists and allied health professionals.
- Identify eligible patients and support proactive patient outreach.
- Build relationships with GPs and nursing teams to increase appropriate patient referrals.
- Maintain accurate clinical records and ensure compliance with Medicare requirements.
- Support other nurses and non-clinical team members in understanding chronic disease management services.
- Contribute to clinical quality, patient safety and continuous improvement.
About You
You are a proactive and compassionate nurse who is passionate about preventative healthcare and supporting patients to better manage their health.
You will bring:
- Current AHPRA registration as a Registered or Enrolled Nurse.
- At least two years’ experience providing direct clinical nursing care is preferred.
- Previous experience within primary healthcare is highly regarded.
- Experience in chronic disease management, health assessments or care planning is preferred.
- Knowledge of health promotion and disease-prevention principles.
- An understanding of Medicare requirements and billing processes.
- Strong written and verbal communication skills.
- Excellent organisation and time-management abilities.
- The confidence to educate, motivate and positively influence patients.
- A collaborative approach to multidisciplinary patient care.
- A genuine interest in improving healthcare quality and patient outcomes.
