As we continue to strengthen and grow our NDIS Program, we are seeking an experienced and proactive Care Coordinator to coordinate participant care, manage clinical and practice risks, support Disability Support Workers and contribute to high-quality and safe service delivery.
This is an excellent opportunity for someone with experience in nursing, social work, allied health, disability or community services who enjoys hands-on care coordination, working with multidisciplinary teams and developing frontline workforce capability.
About the Role
The Care Coordinator plays a key role in ensuring participants receive safe, appropriate and well-coordinated support.
Working closely with the Program Manager and Program Administration team, you will assess participant needs and risks, develop and review support and care plans, coordinate responses to changing needs, monitor clinical and practice risks, and work with internal and external stakeholders to ensure agreed actions are implemented and followed through.
You will also provide practice-level supervision, guidance and development to Disability Support Workers, helping ensure workers understand participant needs, risks and care requirements and have the competency required to provide safe and effective support.
This is a hands-on role suited to someone who takes ownership, exercises sound professional judgement and is comfortable coordinating matters from identification through to implementation and follow-up.
Key Responsibilities
- Coordinate participant intake, assessment, onboarding and service commencement.
- Assess participant needs, risks and support requirements and contribute to service suitability decisions.
- Develop, implement, monitor and review participant support plans, care plans, risk assessments and care arrangements.
- Monitor participant wellbeing and respond to changes in health, behaviour, circumstances, risks or support needs.
- Coordinate participant reviews, case conferences and multidisciplinary meetings where required.
- Manage complex participant care and coordination matters within delegated authority.
- Monitor medication management, incidents, behaviour data, behaviour support, restrictive practices and other clinical or care-related risks.
- Identify when medical, clinical, allied health, Behaviour Support Practitioner or other specialist input is required and coordinate appropriate referrals, consultation and follow-up.
- Ensure professional recommendations are incorporated into participant support arrangements and appropriately implemented.
- Provide practice supervision, guidance, coaching and support to Disability Support Workers.
- Identify worker training and competency requirements and coordinate appropriate training and development.
- Conduct or coordinate competency assessments and follow up identified capability gaps.
- Work collaboratively with participants, families, Support Coordinators, allied health professionals, healthcare providers and other members of participants' care teams.
- Monitor participant documentation, service records and relevant quality and compliance registers.
- Conduct routine participant file, documentation, service and practice audits and coordinate corrective actions.
- Identify recurring care, risk, documentation and practice issues and contribute to continuous quality improvement.
- Work collaboratively with the Program Manager and Program Administration team to support safe, effective and coordinated service delivery.
- Participate in the after-hours on-call roster on a rotating basis.
About You
You are someone who can identify what needs to happen, coordinate the appropriate response and follow matters through to completion.
You are comfortable working directly with participants and frontline workers while also coordinating with families, healthcare professionals and other stakeholders. You understand that effective care coordination involves more than developing plans—it requires ensuring plans are understood, implemented, monitored and adjusted when participant needs change.
You will bring:
- Strong care coordination, case management and problem-solving capability.
- Strong ability to assess participant needs and risks and translate them into safe and practical support arrangements.
- Sound professional judgement and confidence identifying when clinical, specialist or management escalation is required.
- Strong understanding of person-centred practice, participant safeguarding and risk management.
- Ability to provide clear practice guidance, coaching and feedback to Disability Support Workers.
- Strong stakeholder communication and multidisciplinary coordination skills.
- Strong written communication, documentation and record-management skills.
- High attention to detail and commitment to quality.
- The ability to manage multiple participants, competing priorities and complex situations.
- Initiative and a proactive approach to identifying and resolving risks, gaps and outstanding actions.
- A strong sense of ownership and follow-through.
Essential Requirements
- Tertiary qualification in Nursing, Social Work, Allied Health or another relevant health or community services discipline.
- Demonstrated experience in care coordination, case management, clinical coordination or a comparable role within disability, health, aged care or community services.
- Experience developing and reviewing comprehensive support plans, care plans and risk assessments.
- Experience coordinating people with complex or changing support needs.
- Experience supervising, mentoring or providing practice guidance to frontline care or support workers.
- Sound understanding of NDIS service delivery, participant safeguarding and relevant practice requirements.
- Current Australian Driver's Licence.
- National Police Check or willingness to obtain.
- NDIS Worker Screening Check or willingness to obtain.
- Working with Children Check where required or willingness to obtain.
- Current First Aid and CPR certificates or willingness to obtain.
- Full Australian working rights.
- Current professional registration where required for your profession.
Desirable
- Current registration as a Registered Nurse with AHPRA preferred.
- Experience supporting participants with complex disability or high-intensity support needs.
- Experience with medication management, complex health supports, personal care or manual handling.
- Experience with Behaviour Support Plans and restrictive practices.
- Experience conducting participant, documentation or practice audits.
- Experience conducting competency assessments.
- Experience developing or delivering staff training.
- Strong working knowledge of the NDIS Practice Standards and relevant regulatory requirements.
