You will contribute to the success of DPV Health by operating as a Senior Housing Case Manager within the Homelessness team.
Working as part of the Homelessness Team, and reporting to the Team Leader- Outreach, you will be responsible for the provision of a high standard of service to eligible clients in the community.
This will involve you supporting self-management approaches via assessment, care coordination and education to clients with a range of presentations. You will be also assisting with onward referral.
You will, in collaboration with the Team Leader, assist with case consultations, contribute to annual staff work plans, support the management of statistics and error reporting, attend working parties as a representative of the team, and assist in the development and implementation of new initiatives and changes, and contribute to the review and improvement of existing processes, procedures, and policies.

You will have:
- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve housing outcomes as agreed with client.
- Experience working in a community health setting and in outreach including work with some CALD clients and clients with a disability.
- Experience in planning, implementing and evaluating innovative and evidence-based services.
An ability to provide debriefing, secondary consultation, practice guidance, mentoring and training to case managers.
You will also come with
- Relevant Australian tertiary qualification in social work, community services or development, or other equivalent and related discipline;
- Current professional registration with the Australian Health Practitioners Registration Agency (AHPRA), AASW or other relevant body entitling practice in Victoria (if applicable)
The fine print
- Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.
- Full driver license and travel between sites required.
