Are you an experienced disability or community services professional who knows the NDIS sector inside and out?

Do you have strong connections across the disability, aged care and broader community sectors, and a genuine passion for supporting participants to achieve positive outcomes?

Your Care Co is looking for an experienced Team Leader – Workforce & Service Delivery to join our growing team in Geelong.

This is an opportunity for someone who can combine strong NDIS knowledge, people leadership, workforce coordination and participant-focused service delivery to make a real difference.

About the role

You will play a key role in coordinating and leading the day-to-day delivery of participant services while supporting and developing a skilled, engaged and compliant workforce.

You will work closely with participants, families, Support Coordinators, brokerage partners, allied health professionals and community organisations to ensure high-quality, consistent and person-centred supports.

We are looking for someone who:

Has extensive experience within the NDIS and disability sector
Has strong knowledge of participant care and person-centred service delivery
Has established connections and networks within the disability, aged care and community sectors
Can identify and develop referral and brokerage opportunities
Has experience leading, mentoring, coaching and supporting frontline teams
Is confident managing workforce planning, rostering and service coordination
Has experience recruiting, onboarding and retaining quality support workers
Can build strong, trusted relationships with participants, families and stakeholders
Understands NDIS compliance, Practice Standards and quality requirements
Is a confident communicator who can problem-solve, make decisions and lead from the front

You will be someone who can mentor and coach staff, build workforce capability, foster accountability and create a positive team culture, while keeping participant outcomes at the centre of everything you do.

Your existing industry relationships and community connections will be highly valued as you help strengthen referral pathways, develop brokerage relationships and contribute to the growth of Your Care Co.
