About the role

As a Senior Social Worker, you will provide advanced, evidence-informed social work services to patients, families and carers across Surgical, Cancer Care and Palliative Care services at Redcliffe Hospital. Working within a multidisciplinary team, you will deliver comprehensive psychosocial assessment, counselling, advocacy, care coordination and complex discharge planning to support patients with a wide range of psychosocial, adjustment, safeguarding and supportive care needs.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Apply high-level understanding of the environment in which clinical decisions are made to influence health outcomes, including a broad understanding of the continuum of care and the organisational provision of multidisciplinary health services.
Apply high level clinical practice to support continuous improvement of quality service delivery including contributing to the development of evidence based clinical practices, procedures, and guidelines within the Social Work Department to facilitate optimal client outcomes.
Deliver high level clinical and operational advice and work collaboratively and effectively with other Social Work practitioners, professional and operational supervisors, relevant service managers and other stakeholders regarding social work service delivery and the prioritisation and development of clinical services.
Demonstrate highly developed skills in advocacy, system navigation, negotiation, and conflict management, with a proven ability to engage and influence patients, families, healthcare professionals, and external agencies to achieve safe, person-centred, and sustainable outcomes across the continuum of care.
