About the role
As the Senior Social Worker - Internal Medicine, you will play a key role in delivering advanced social work services to patients, families and carers within a busy tertiary hospital environment at the Royal Brisbane and Women's Hospital. Working as part of a highly skilled multidisciplinary team, you will provide comprehensive psychosocial assessment, risk management, counselling, advocacy and discharge planning for patients with complex care needs. Your expertise will support safe, timely and patient-centred care, while contributing to improved patient flow and service delivery outcomes across the Internal Medicine Stream.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Deliver high quality, evidence based clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards by assessing complex psychosocial needs of patients, families, and caregivers; identifying clinical risk and providing multiple clinical interventions to maximise patient health and psychosocial outcomes.
Possess a high level of knowledge, skills and experience in working with Residential Aged Care, QCAT and NDIS pathways.
Work cooperatively and effectively with patients, their families, and caregivers, as a member of a multidisciplinary and social work teams.
Provide high level expertise in the delivery of social work services to clients through collaborative participation as a multidisciplinary team member and contribute towards multidisciplinary team processes.
Contribute to the ongoing identification, development and implementation of quality and service improvement activities that enhance the delivery of social work services and patient care.
