About the role
The purpose of the role is to provide direct clinical social work services include assessment, planning and delivery of care to inpatients at Ipswich Hospital under Acute Medicine.
Working within a multidisciplinary team environment, the role will provide interventions to meet the psychosocial care needs for an increasingly complex clinical caseload of adults to promote, maintain and improve their health, functional ability and quality of life.
The Social Worker will collaborate with appropriate inpatient teams and community health providers and community agencies to help meet patient needs that will facilitate best possible health outcomes and support the continuum of care.

Mandatory Requirements
Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association within Australia for a Social Worker, eligibility of AASW membership. Certified copies of the required information must be provided to the appropriate supervisor/ manager, prior to the commencement of clinical duties.
Ability to undertake work on the Social Work, After Hours on call roster with the requirement of 7 days a week rostering availability.
It is a condition of employment for this role for the employee to be, and remain, vaccinated against the following vaccine preventable diseases during their employment as required by HED 01/16
