At Concentric, we’re redefining what allied health in aged care looks like. Instead of purely maintenance-based care, our teams deliver proactive, outcomes-focused therapy through dedicated onsite Active Living Program that help our clients thrive.

You’ll work in a model that goes beyond standard aged care treatment, delivering tailored Active Living Program, structured rehabilitation programs, and engaging group exercise and fitness sessions designed to improve strength, mobility, balance, cognition, and overall quality of life.

Our clinicians partner closely with residential teams while also supporting community clients, giving you the chance to run groups, provide targeted 1:1 therapy, and see real functional progress in your clients.

The role unwrapped:

Assess the residents: to understand their functional abilities, goals, and challenges, identifying opportunities to improve mobility, safety, and participation in daily activities.
Develop personalised therapy: that enhance independence, build strength and confidence, and support residents to live well through targeted rehabilitation, group programs, and meaningful functional activities.
Collaborate: Team up with residents, families, and colleagues for person-centred care.
Document: Keep documentation clear, simple, and spot-on.

Engage & thrive: Join meetings, case chats, and growth opportunities—while having fun with the team.
