The Team & Role
The Single Assessment System (SAS) streamlines aged care access by using one eligibility process for services such as CHSP, Home Care Packages, and residential care. It includes both Comprehensive Assessments, which provide a holistic evaluation of an individual’s physical, medical, psychological, and social needs to develop tailored support plans, and Home Support Assessments, which focus on goal setting and reablement approaches for clients living at home. Assessors conduct evaluations, create support plans, generate referrals through My Aged Care, and coordinate services to promote independence, while maintaining required competencies and contributing to continuous service improvement.

Skills & Experience Required
- Bachelor Degree in allied health qualification, registered with APHRA
- Understanding and / or experience working in the acute, rehabilitation or community services for the aged
- Demonstrated knowledge of and experience in the provision of Primary Care services especially in respect of assessment and service delivery.
- Demonstrates both short and longer term care planning for clients
- Excellent written and verbal communication skills.
- Postgraduate studies Community Health or Gerontology
