Gateway Health is a not-for-profit community health service delivering primary health and welfare services to individuals, families, and communities across Northeast Victoria and parts of Southern New South Wales. About the Role
This is a rewarding full time temporary opportunity (76 hours per fortnight until 30th June 2027) based in Wangaratta.
In this outreach-focused role, you will join our Care Finder team as a Regional Health Navigator, supporting clients across the North East region. You will work alongside individuals who may experience barriers navigating the health and community care system, helping them access the services and supports they need to improve their health and wellbeing.
Using a person-centred and strengths-based approach, you'll support clients to understand available care options, referral pathways, and local services while building their confidence to engage with healthcare, community, and social support systems. You'll also collaborate with health professionals, community organisations, and service providers to strengthen connections and improve access to care across the region.
This role is classified under the Community Health Centre Stand Alone Services Social and Community Service Employees Multi Enterprise Agreement 2022 (SACS) at Level 3 ($38.65 - $41.45 per hour + superannuation).
Skills & Experience
To thrive in this role, you'll bring:
- Qualification (minimum Certificate IV level) in Health, Community Services, Community Development, or a related discipline.
- Demonstrated skills and experience working with people with a chronic disease to help them navigate the healthcare system.
- Excellent organisation and time management skills, including the ability to work autonomously
