What your day could look like
As a Senior Pharmacist, you'll play a critical role in supporting older people to stay safe, well and independent in the community. From shaping medication plans that reduce hospital visits to working virtually and in-home with patients, you'll collaborate with a multidisciplinary team to deliver care that truly makes a difference.
Every day, you'll contribute by:
Designing and delivering medication management strategies that improve safety and reduce avoidable hospital admissions
Partnering with nurses, allied health and medical teams to support clinical decisions and coordinated care
Leading innovative models of care, including telehealth and hospital-in-the-home services
Strengthening continuity of care by working with primary providers and supporting smooth transitions home

You're a capable and compassionate pharmacist who thrives in complex care environments and values making a meaningful difference. You combine clinical expertise with strong communication, bringing people together to make safe, thoughtful decisions for older patients. You take pride in helping people maintain independence and dignity through better medication use, while embracing new ways of working to improve outcomes.
Here's what will help you thrive in this role:
Current registration as a pharmacist with AHPRA and relevant tertiary qualifications
Strong clinical pharmacy experience, ideally in aged care or complex patient groups
Confident communicator who can influence, collaborate and build trusted relationships
Ability to work both independently and within multidisciplinary teams in dynamic settings
You'll stand out from the crowd if you have experience in rural or community care models and/or additional training in geriatric pharmacy or quality use of medicines. Most importantly, you're passionate about improving care for older people and ready to embrace the opportunities of regional practice.
In this role, your expertise and care will help older people live safer, healthier lives while reflecting the values of compassion, integrity, dignity, innovation and courage.
