Support Coordinator/ Specialist Support Coordinator

The Role:

As a Specialist Support Coordinator / Support Coordinator you will bring compassion and experience to the role to assist participants to build their capacity so they feel confident and are able to make decisions and choices about the supports in their NDIS plans and the way these are implemented. This includes all Mainstream, Community, Informal, and funded supports. The Specialist Support Coordinator / Support Coordinator will connect participants with an NDIS plan, to a range of different supports that align with their NDIS goals and enables them to live a full meaningful life.

The Successful applicants

Will be required to travel to meet with both participants and providers in the NSW Metropolis region and surrounds

Key Selection Criteria

Satisfactory pre-employment screening checks are required for the role including DCSI checks: Working with Children Check, NDIS Worker Screening Check and Referee check.
Relevant tertiary qualifications with a minimum 1 years relevant experience, case management, support coordination or disability experience preferred. Social work degree or recognised prior experience highly recommended.
Excellent practical and theoretical knowledge of people with disabilities as well as a good knowledge of complex and multiple care needs. This will include undertaking risk assessments and being able to provide plans to mitigate risk.
Demonstrated skills and knowledge of NDIS plans, funding and implementation as well as NDIS Act 2013 and NDIS Rules 2013. This includes adhering to the NDIS Practice Standards.
Understanding of the NDIS Pricing Arrangements and the flexibility between budgets of an NDIS plan.
Demonstrated knowledge and ability to work with a client focused environment to build client capacity and provide choice and control to manage their NDIS plans.
Acquired sound local knowledge of service providers and the ability to find services and supports to meet the clients needs.
High level of report writing skills is essential.
A current South Australian Drivers Licence and ability to undertake travel is essential for this role.
Current First Aid and CPR credentials desirable but not essential
