About the Role
Join our community-based District Nursing team and support consumers to achieve their health goals at home. This role delivers holistic, consumer-centred nursing care across a range of funded community programs. You’ll work autonomously and collaboratively to provide safe, effective and evidence-based care that makes a real difference.

Key Responsibilities
- Assess, plan and deliver evidence-based nursing care
- Coordinate care with consumers, carers and teams
- Respond to changing clinical needs and priorities
- Maintain accurate clinical documentation
- Uphold infection control and safety standards
- Participate in quality improvement activities
Skills & Experience
- Registered Nurse with current AHPRA registration
- Advanced clinical skills in community or acute care
- Strong assessment and care planning capability
- Effective communication and teamwork skills
- Current Australian Driver’s Licence

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is District Nurse Level 2.3 (YQ3) to District Nurse Level 2 (YU1) which ranges from $50.76 - $55.07 per hour.
