Position: Community Worker

Salary: $77,000-$90,000 per annum plus superannuation.

Care and Concern Australia is a registered NDIS provider delivering personalised disability and community support services across Queensland. We are committed to promoting independence, inclusion, and quality of life through compassionate and person-centred care. We are currently seeking a dedicated Community Worker to support participants in achieving their individual goals and community engagement outcomes.

Duties and Responsibilities:

Deliver person-centred community support services to individuals with disability in accordance with individual goals, support plans, and NDIS requirements.
Assist participants to access community, educational, recreational, employment, health, and social support services to encourage independence, wellbeing, and social inclusion.
Support individuals to develop life skills, communication abilities, decision-making capacity, confidence, and community participation through structured support and engagement activities.
Identify participant support needs and assist with referrals to appropriate healthcare, welfare, housing, counselling, mental health, and community support services.
Work collaboratively with families, carers, allied health professionals, support coordinators, and community organisations to promote continuity of support and positive participant outcomes.
Encourage participants to actively engage in social, cultural, recreational, and community development activities that promote healthy and independent living.
Monitor participant wellbeing, behavioural changes, and progress toward individual goals, maintaining accurate case notes, incident reports, and service documentation in accordance with organisational and NDIS standards.
Assist in implementing individual support strategies, behaviour support practices, recovery-focused approaches, and risk management procedures while ensuring participant dignity, safety, and cultural respect.
Advocate for participants and vulnerable community members to improve access to services, strengthen community connections, and promote inclusion for people with disability from diverse cultural and linguistic backgrounds.
Participate in community engagement initiatives, participant advocacy programs, and organisational activities designed to improve quality of life, social participation, and community awareness.

Relevant skills:

AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2).
At least three years of relevant experience may substitute for the formal qualification.
Knowledge of NDIS practices, participant rights, and person-centred care principles.
Ability to communicate effectively with clients, families, and multidisciplinary teams.
Strong organisational, reporting, and interpersonal skills.
Capability to work independently and within multicultural community environments.

Pay: $77,000.00 – $90,000.00 per year
