About the Role
We are seeking a dedicated Care Partner to join our team. In this role, you will work closely with the Registered Nurse (RN) and Care Coordinator to deliver and monitor Support at Home services.
You will play a key role in coordinating care, supporting clients to achieve their goals, and ensuring services are safe, effective, and aligned with individual needs. The position requires a strong person-centred approach and the ability to work collaboratively within a multidisciplinary team.
The role involves coordinating supports to enable older people to live independently at home, while promoting wellness, reablement, and informed choice.
Key ResponsibilitiesCare Coordination & Service Delivery
- Coordinate and review services to ensure supports remain appropriate and effective
- Assist clients to access and navigate Support at Home services
- Support integration of Assistive Technology and home modifications where required.
Care Planning & Monitoring
- Assist in developing and reviewing individualised care plans aligned with client goals
- Monitor client progress, identify risks, and escalate concerns to the RN or Care Coordinator
- Implement and follow clinical recommendations as directed
Collaboration & Communication
- Work closely with the RN and Care Coordinator to deliver coordinated care
- Liaise with families, support workers, and external health professionals
- Provide education and guidance to clients to support independence and self-management
Documentation & Compliance
- Maintain accurate and timely documentation in line with aged care standards
- Support reporting, record-keeping, and service tracking requirements
Service Quality & Continuous Improvement
- Contribute to quality improvement initiatives and compliance processes
- Participate in team meetings and ongoing professional development
About YouEssential Skills & Experience
- Experience in aged care, community care, or Support at Home/Home Care Packages
- Strong communication and organisational skills
- Ability to work collaboratively with multidisciplinary teams and independently
- Demonstrated ability to build respectful relationships with clients and families
Qualifications (Desirable)
- Certificate III or IV in aged care, Certificate IV in Disability, Enrolled Nurse, Registered Nurse, Allied Health qualification, or relevant industry experience in aged care
Mandatory Requirements
- Current National Police Check
- Working with Children Check
- NDIS Worker Screening Check
- Valid Victorian Driver’s Licence
