POSITION DETAILS
This role supports adults with severe mental illness who are at risk of homelessness to achieve recovery and housing stability outcomes, in line with Footprints service model and in collaboration with Metro North and Metro South Hospital and Health Services. The Recovery and Wellness Program provides tailored psychosocial support to promote wellbeing, independence and long term stability.
RESPONSIBILITIES

Develop comprehensive person centred strengths based needs assessments and tailored recovery action plans including crisis management strategies to optimise client wellbeing.

Provide specialised non clinical care coordination and direct outreach support to individuals with complex needs to build capacity for self management and self determination.

Identify and assess risk utilising established Queensland Health and Footprints matrices to ensure the highest standards of safe practice.

Deliver supportive counselling and harm minimisation coaching for substance use, emotional regulation, problem solving, and conflict management.

Offer expert mentoring and supervisory opinions to caseworkers to ensure best practice standards and foster a culture of hope and optimism.

Collaborate with Queensland Health Clinical teams, Hospital and Health Services, housing networks, and primary care providers while facilitating access to NDIS and My Aged Care.
KEY SELECTION CRITERIA
Essential
-
Relevant tertiary qualifications in social science, allied health, community services, or a similar discipline, alongside extensive experience in complex mental health environments.

Proven ability to deliver recovery oriented service models and oversee recovery focused approaches for individuals living in marginalised accommodation settings.
Demonstrated capability to show initiative, think critically, make informed ethical decisions, and develop risk mitigation strategies to ensure safe practice.
Well developed written and verbal communication skills with a proven capacity to build strong relationships with key internal and external stakeholders.
Strong understanding of NDIS and My Aged Care application processes combined with proficiency in managing client databases and producing reports.
Possession of a current class C drivers licence, access to a comprehensively insured personal vehicle, and a valid National NDIS Worker Screening Check/ Yellow Card or willing to obtain one.
