About the role:
The primary purpose of this position is to support the delivery of the Footprints Aged Care Assessment Service (ACAS) through the provision of holistic aged care assessments. The substantive role of this position is Clinical Assessor, with potential additional responsibilities in Triage Delegate and Assessment Delegate functions where the employee has completed the required training, certification, and authorisation, and where operational needs require these functions to be performed.
This hybrid clinical role supports responsive, client-centred, comprehensive, fair, and timely assessment pathways for older Australians accessing Commonwealth-funded aged care services. Staff may work across one or more of these functions based on operational requirements, workforce capability, delegation status, and service demand.
The role is responsible for undertaking aged care needs assessments, determining eligibility for assessment services, reviewing assessment outcomes and support plan recommendations, and exercising delegated authority under the Aged Care Act in accordance with legislative requirements, operational guidance, program expectations, and approved delegation arrangements.

In this role, you will:

Complete Comprehensive Home Support and/or Comprehensive Assessments using the Integrated Assessment Tool and other validated assessment tools where required.

Conduct assessments in line with the My Aged Care Assessment Manual, Aged Care Assessment Quality Framework, program guidelines, contractual timeframes, and quality requirements.
- Facilitate client access to Commonwealth-funded aged care services, state-based supports, allied health services, medical services, and community programs.
- Work collaboratively with clients, carers, representatives, service providers, and stakeholders to support coordinated and person-centred outcomes.
- Ensure clients are linked to relevant supports and follow up where appropriate to support continuity of care and service engagement.
- Lead the triage process for referrals by reviewing and gathering information, determining eligibility, determining assessment type and priority level, issuing urgent referrals where required, allocating assessments, and exercising duties as a delegate under the Act with due care.

Review aged care needs assessments and approve recommendations and support plans as a delegate under the Act.
- Ensure delegate decisions are evidence-based, appropriately documented, and consistent with legislative requirements, operational guidance, and delegated authority.
- Complete Notices of Decision accurately, objectively, and in accordance with legislative requirements, assessment evidence, and organisational procedures

Support the implementation of aged care reforms and assist clients and stakeholders to understand the aged care assessment pathway and access to Government-funded services.
- Support coordinated and transparent service delivery through the centralised client record and My Aged Care Assessor Portal.
About you:
To be successful, you will bring:

Tertiary qualifications as a Registered Nurse, Occupational Therapist, or Physiotherapist with current AHPRA registration, or Social Worker with eligibility for AASW membership.

Demonstrated experience working with older people and their families/carers, including individuals experiencing mental illness, disability, homelessness, housing instability, or social and financial disadvantage.
- Demonstrated understanding and experience in delivering community-based aged care services using a wellness and reablement approach, including assessment, care planning, monitoring, and evaluation processes.
- Experience undertaking assessments, triage, care coordination, and goal-orientated support planning within aged care, community, health, or human services settings.
- Demonstrated ability to exercise sound clinical judgement, decision-making, and problem-solving skills within legislative and operational frameworks.
- Demonstrated knowledge of aged care reforms, the My Aged Care assessment pathway, and person-centred service delivery principles.
- High level computer literacy and experience working across multiple digital platforms and systems, including Microsoft Office Suite and client management systems.
- Local knowledge and/or the ability to build networks with community aged care providers and stakeholders within the region.
- Current Class C Driver Licence.
- Current NDIS Worker Screening Check and/or Yellow Card.
- Evidence of annual influenza vaccination in accordance with Department of Health and Aged Care requirements.
Desirable:

Previous experience working within My Aged Care, aged care assessment services, delegate functions, or care coordination roles.

Current First Aid and CPR Certificate.

Experience working within hybrid, remote, or regionally based service delivery models.
