About the role

The Nurse Practitioner (NP) - Residential Aged Care Acute Support Service (RaSS) is an advanced practice clinician who provides hospital-level acute care substitution, clinical leadership and system navigation for residents within Residential Aged Care Homes (RACH) across West Moreton Health. The NP practices autonomously within a collaborative multidisciplinary model, delivering timely, evidence based assessment, diagnosis and management of acute deterioration in RACH setting.

About you

Our ideal candidate will be someone who can demonstrate the following:

Demonstrated advanced clinical expertise in assessment, diagnosis, and management of acute deterioration in older persons within RACH or equivalent settings
Proven ability to operate autonomously and make high-level clinical decisions in complex, non-hospital environments.
Demonstrated experience in acute care substitution and hospital avoidance models of care.
Strong clinical reasoning, risk assessment, and escalation decision-making capability.
Demonstrated ability to build effective relationships and influence care across multidisciplinary teams, RACH staff, and primary care providers.
High-level communication skills, including ability to lead goals-of-care discussions and manage complex stakeholder conversations.

Mandatory Requirements:

Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
