About the Role
We have an opportunity available for a relationship-driven Senior Case Manager to join our Mornington team, supporting older Australians in the region to live independently, safely and confidently in their own homes.
This is a varied and rewarding role that combines office-based care coordination with face-to-face client interaction. You’ll partner closely with clients and families to understand their goals, determine their care team, manage funding and budgets, and adapt services as client needs change over time. In addition, you will also coach and mentor a small team of Case Managers.
Working across privately funded clients and those who receive government funded Support at Home Packages, you’ll oversee the full case management journey, from intake and onboarding through to ongoing care reviews, funding management and service coordination. You’ll also collaborate with allied health providers and other external stakeholders to ensure every client receives high-quality, personalised support.
This is a 12-month maximum term contract to cover parental leave.
About You
You enjoy building genuine relationships, thrive in a fast-paced environment and bring a calm, organised and solutions-focused approach to your work. Passionate about supporting older people to live well at home, you're highly organised, adaptable and capable of managing competing priorities, funding and budgeting requirements, and navigating complex situations.
You will bring:
- Knowledge of Support at Home Funding and experience managing related client budgets
- Case Management and Care Planning experience
- Clinical knowledge and confidence managing changing client needs and complex situations
- Excellent time management and organisational skills
- Exceptional communication and relationship-building capability
- Experience leading a small team desirable
- Willingness to travel for client visits across the community
An Allied Health background, particularly Occupational Therapy, will be highly regarded,
as will be a qualification in Aged Care, Community Services, Disability or Nursing.
