About the Role
We are seeking an experienced and compassionate Senior Community Support Worker – NDIS Complex Mental Health to join our growing team.
This role involves supporting NDIS participants experiencing psychosocial disability, complex mental health challenges, and barriers to community participation. The successful candidate will work collaboratively with participants, families, carers, service providers, and community stakeholders to build capacity, strengthen independence, improve wellbeing, and achieve NDIS goals through recovery-oriented and person-centred support.
This position involves providing community-based support to NDIS participants across Sydney's Eastern Suburbs and surrounding areas. Travel between participant locations is required; therefore, a current driver's licence and reliable insured vehicle are mandatory.
This position offers a professional development pathway to become a Psychosocial Recovery Coach for suitable candidates who demonstrate the required skills, qualifications, and commitment to recovery-focused practice.
Key Responsibilities
- Meet with participants to understand their circumstances and identify the types of support they may require.
- Develop and deliver tailored support plans, workshops, recovery-focused activities, and programs that respond to participant needs.
- Support participants to build independence, improve daily living skills, strengthen community connections, and achieve NDIS goals.
- Assist participants with community access, appointments, transport, social participation, and life skills development.
- Monitor participant progress and maintain accurate case notes, updates, reports, and participant documentation.
- Connect participants with external organisations, health professionals, community services, housing supports, employment providers, and other services that can offer additional assistance.
- Review local community needs, available services, and referral pathways to support participant outcomes and guide service planning.
- Build relationships with community groups, service providers, government agencies, allied health professionals, local businesses, families, and carers to share information and promote available supports.
- Prepare written material such as participant reports, program summaries, progress updates, funding-related documentation, and reports for internal and external stakeholders.
- Lead and support community-focused projects, workshops, and activities that bring people together to create practical solutions for challenges relating to recreation, health, housing, employment, education, and social wellbeing.
- Advocate for participant choice, control, inclusion, and access to community opportunities.
- Support participants experiencing complex mental health challenges using recovery-oriented, strengths-based, and trauma-informed approaches.
- Identify and manage risks, escalating concerns where required and maintaining participant safety.
- Work collaboratively within a multidisciplinary team to achieve positive participant outcomes.
- Maintain compliance with NDIS Practice Standards, NDIS Quality and Safeguards requirements, and organisational policies and procedures.
Essential Qualifications and Experience
- Diploma or Advanced Diploma in Community Services, Welfare, Social Services, or a related field.
- Minimum one year experience working in community services, case management, welfare support, psychosocial support, or mental health support.
- Strong communication and interpersonal skills with the ability to work effectively with people from diverse backgrounds.
- Understanding of local support systems, referral pathways, and community resources.
- Demonstrated ability to build rapport and maintain professional relationships with participants, families, and stakeholders.
- Strong organisational, time management, and documentation skills.
Mandatory Requirements
- Current NDIS Worker Screening Check.
- Current Working With Children Check (WWCC).
- Current First Aid and CPR Certificate.
- Current National Police Check (within the last 12 months).
- NDIS Worker Orientation Module Certificate – Quality, Safety and You.
- Current unrestricted Australian Driver's Licence.
- Reliable, registered, and comprehensively insured motor vehicle.
- Current right to work in Australia.
- Ability to travel throughout Sydney's Eastern Suburbs and surrounding areas.
Desirable
- Experience supporting people with psychosocial disability or complex mental health challenges.
- Mental Health First Aid Certificate.
- Medication Administration Training.
- Knowledge of NDIS Practice Standards and NDIS Quality and Safeguards requirements.
- Experience facilitating groups, workshops, or community-based programs.
- Understanding of recovery-oriented practice and trauma-informed care principles.
