Full Job Description

Atticus Health is seeking a compassionate and adaptable Home Care Partner / Practice Nurse to join our team in a full-time role supporting the Palm Lake Resort Paynesville community and surrounding home care clients.

This unique role combines face-to-face nursing support with facilitating telehealth consultations when the GP is not onsite. The GP will provide face-to-face consultations within the community 2 days per week.

About the Role

Support residents within the Palm Lake Resort Paynesville community
Facilitate telehealth consultations between patients and the GP
Assist with face-to-face GP consultations when the GP is onsite
Provide clinical nursing care including observations, wound care, health assessments, chronic disease management, and care coordination
Support home care clients within the local area
Maintain accurate clinical documentation and communication with the healthcare team
Deliver compassionate, patient-centred care

About You

Current AHPRA registration as a Registered Nurse or Enrolled Nurse
Experience in general practice or home care preferred
Strong communication and organisational skills
Comfortable using technology and telehealth systems
Ability to work independently and collaboratively
Current driver’s licence and reliable vehicle preferred
